Office Hours
Our office hours are Monday through Friday: 8:30am to 5:00pm. We are often unable to answer the phone from 1:00pm until 2:00pm – please try to avoid calling for non-emergent issues during that time as your call may be directed to our live answering service. Our doctors are on call for emergencies 24 hours a day. For general information or to schedule an appointment, please contact one of our two convenient locations below.

Payment Options
Payment is expected at the time of visit by Cash, Check, or Charge (Visa, MasterCard, Discover, and American Express) for all out of pocket expenses.

The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time, you will save a significant amount of time during your visit.

What to Bring to Your First Appointment
- Completed patient information form
- Primary Doctor’s name, address, telephone# and fax#
- Pharmacy info
- Insurance Cards
- Referral if needed
- Current Glasses
- Contact lens wearers please bring: brand of C/L and current RX
- List of current medications (prescription and non-prescription)
Cancellation Policy
Please notify the office at least 24-48 hours prior to your scheduled visit if you need to cancel or change your appointment.
